3fb87d13ce nfsd4: hash deleg stateid only on successful nfs4_set_delegation
We don't want the stateid to be found in the hash table before the delegation
is granted.

Currently this is protected by the client_mutex, but we want to break that
up and this is a necessary step toward that goal.

ba5378b66f nfsd: fix setting of NFS4_OO_CONFIRMED in nfsd4_open
In the NFS4_OPEN_CLAIM_PREVIOUS case, we should only mark it confirmed
if the nfs4_check_open_reclaim check succeeds.

In the NFS4_OPEN_CLAIM_DELEG_PREV_FH and NFS4_OPEN_CLAIM_DELEGATE_PREV
cases, I see no point in declaring the openowner confirmed when the
operation is going to fail anyway, and doing so might allow the client
to game things such that it wouldn't need to confirm a subsequent open
with the same owner.

931ee56c67 nfsd4: use recall_lock for delegation hashing
This fixes a bug in the handling of the fi_delegations list.

nfs4_setlease does not hold the recall_lock when adding to it. The
client_mutex is held, which prevents against concurrent list changes,
but nfsd_break_deleg_cb does not hold while walking it. New delegations
could theoretically creep onto the list while we're walking it there.

beba4bb096 tracing: Add __get_dynamic_array_len() macro for trace events
If a trace event uses a dynamic array for something other than a string
then there's currently no way the TP_printk() can figure out what size
it is. A __get_dynamic_array_len() is required to know the length.

This also simplifies the __get_bitmask() macro which required it as well,
but instead just hardcoded it.

cad6d02acc IB/core: Fix port kobject deletion during error flow
When encountering an error during the add_port function, adding a port
to sysfs, the port kobject is freed without being deleted from sysfs.

Instead of freeing it directly, the patch uses kobject_put to release
the kobject and delete it.

373c0ea181 IB/core: Remove unneeded kobject_get/put calls
The ib_core module will call kobject_get on the parent object of each
kobject it creates.  This is redundant since kobject_add does that
anyway.

As a side effect, this patch should fix leaking the ports kobject and
the device kobject during unregister flow, since the previous code
didn't seem to take into account the kobject_get calls on behalf of
the child kobjects.

 "It is very late but this is an important percpu-refcount fix from
  Sebastian Ott.

  The problem is that percpu_ref_*() used __this_cpu_*() instead of
  this_cpu_*().  The difference between the two is that the latter is
  atomic on the local cpu while the former is not.  this_cpu_inc() is
  guaranteed to increment the percpu counter on the cpu that the
  operation is executed on without any synchronization; however,
  __this_cpu_inc() doesn't and if the local cpu invokes the function
  from different contexts (e.g.  process and irq) of the same CPU, it's
  not guaranteed to actually increment as it may be implemented as rmw.

  This bug existed from the get-go but it hasn't been noticed earlier
  probably because on x86 __this_cpu_inc() is equivalent to
  this_cpu_inc() as both get translated into single instruction;
  however, s390 uses the generic rmw implementation and gets affected by
  the bug.  Kudos to Sebastian and Heiko for diagnosing it.

  The change is very low risk and fixes a critical issue on the affected
  architectures, so I think it's a good candidate for inclusion although
  it's very late in the devel cycle.  On the other hand, this has been
  broken since v3.11, so backporting it through -stable post -rc1 won't
  be the end of the world.

  I'll ping Christoph whether __this_cpu_*() ops can be better annotated
  so that it can trigger lockdep warning when used from multiple
  contexts"

should also be converted to this_cpu_ince().

This commit pulls in percpu/for-3.15-fixes and converts the newly
added __this_cpu_inc() to this_cpu_inc().

14b83e172f block: mq flush: clear flush_rq's tag in flush_end_io()
blk_mq_tag_to_rq() needs to be able to tell if it should return
the original request, or the flush request if we are doing a flush
sequence. Clear the flush tag when IO completes for a flush, since
that is what we are comparing against.

0e62f51f87 blk-mq: let blk_mq_tag_to_rq() take blk_mq_tags as the main parameter
We currently pass in the hardware queue, and get the tags from there.
But from scsi-mq, with a shared tag space, it's a lot more convenient
to pass in the blk_mq_tags instead as the hardware queue isn't always
directly available. So instead of having to re-map to a given
hardware queue from rq->mq_ctx, just pass in the tags structure.

0c36b390a5 percpu-refcount: fix usage of this_cpu_ops
The percpu-refcount infrastructure uses the underscore variants of
this_cpu_ops in order to modify percpu reference counters.
(e.g. __this_cpu_inc()).

However the underscore variants do not atomically update the percpu
variable, instead they may be implemented using read-modify-write
semantics (more than one instruction).  Therefore it is only safe to
use the underscore variant if the context is always the same (process,
softirq, or hardirq). Otherwise it is possible to lose updates.

This problem is something that Sebastian has seen within the aio
subsystem which uses percpu refcounters both in process and softirq
context leading to reference counts that never dropped to zeroes; even
though the number of "get" and "put" calls matched.

Fix this by using the non-underscore this_cpu_ops variant which
provides correct per cpu atomic semantics and fixes the corrupted
reference counts.

27423257b7 ALSA: seq: Continue broadcasting events to ports if one of them fails
Sometimes PORT_EXIT messages are lost when a process is exiting.
This happens if you subscribe to the announce port with client A,
then subscribe to the announce port with client B, then kill client A.
Client B will not see the PORT_EXIT message because client A's port is
closing and is earlier in the announce port subscription list. The
for each loop will try to send the announcement to client A and fail,
then will stop trying to broadcast to other ports. Killing B works fine
since the announcement will already have gone to A. The CLIENT_EXIT
message does not get lost.

When the code was collapsed to avoid duplication, the recent patch
for ensuring that a queue is idled before free was dropped, which was

Pull NFS client side changes for RDMA from Anna Schumaker

These patches are mostly cleanups and bugfixes for using RDMA as an
over-the-wire transport.

Highlights include:

- Remove obsolete memory registration modes.
- Removing BUG_ON()s to keep client's running.
- Fix deadlocks, NULL-pointer dereferences, and memory leaks.

11f0431be2 dm: remove symbol export for dm_set_device_limits
There is no need for code other than DM core to use dm_set_device_limits
so remove its EXPORT_SYMBOL_GPL.  Also, cleanup a couple whitespace nits.

7eee4ae2db dm: disable WRITE SAME if it fails
Add DM core support for disabling WRITE SAME on first failure to both
request-based and bio-based targets.  The need to disable WRITE SAME
stems from SCSI enabling it by default but then disabling it when it
fails.  When SCSI does this it returns "permanent target failure, do
not retry" using -EREMOTEIO.  Update DM core to only disable WRITE SAME
on failure if the returned error is -EREMOTEIO.

Commit f84cb8a4 ("dm mpath: disable WRITE SAME if it fails")
implemented multipath specific disabling of WRITE SAME if it fails.
However, as that commit detailed, the multipath-only solution doesn't go
far enough if bio-based DM targets are stacked ontop of the
request-based dm-multipath target (as is commonly done using dm-linear
to support partitions on multipath devices, via kpartx).

c93c62231c xprtrdma: Disconnect on registration failure
If rpcrdma_register_external() fails during request marshaling, the
current RPC request is killed. Instead, this RPC should be retried
after reconnecting the transport instance.

The most likely reason for registration failure with FRMR is a
failed post_send, which would be due to a remote transport
disconnect or memory exhaustion. These issues can be recovered
by a retry.

Problems encountered in the marshaling logic itself will not be
corrected by trying again, so these should still kill a request.

Now that we've added a clean exit for marshaling errors, take the
opportunity to defang some BUG_ON's.

e7ce710a88 xprtrdma: Avoid deadlock when credit window is reset
Update the cwnd while processing the server's reply.  Otherwise the
next task on the xprt_sending queue is still subject to the old
credit window. Currently, no task is awoken if the old congestion
window is still exceeded, even if the new window is larger, and a
deadlock results.

This is an issue during a transport reconnect. Servers don't
normally shrink the credit window, but the client does reset it to
1 when reconnecting so the server can safely grow it again.

As a minor optimization, remove the hack of grabbing the initial
cwnd size (which happens to be RPC_CWNDSCALE) and using that value
as the congestion scaling factor. The scaling value is invariant,
and we are better off without the multiplication operation.

18906972aa xprtrdma: Reset connection timeout after successful reconnect
If the new connection is able to make forward progress, reset the
re-establish timeout. Otherwise it keeps growing even if disconnect
events are rare.

The same behavior as TCP is adopted: reconnect immediately if the
transport instance has been able to make some forward progress.

196c69989d xprtrdma: Allocate missing pagelist
GETACL relies on transport layer to alloc memory for reply buffer.
However xprtrdma assumes that the reply buffer (pagelist) has been
pre-allocated in upper layer. This problem was reported by IOL OFA lab
test on PPC.

ec62f40d35 xprtrdma: Ensure ia->ri_id->qp is not NULL when reconnecting
Devesh Sharma <Devesh.Sharma@Emulex.Com> reports that after a
disconnect, his HCA is failing to create a fresh QP, leaving
ia_ri->ri_id->qp set to NULL. But xprtrdma still allows RPCs to
wake up and post LOCAL_INV as they exit, causing an oops.

rpcrdma_ep_connect() is allowing the wake-up by leaking the QP
creation error code (-EPERM in this case) to the RPC client's
generic layer. xprt_connect_status() does not recognize -EPERM, so
it kills pending RPC tasks immediately rather than retrying the
connect.

Re-arrange the QP creation logic so that when it fails on reconnect,
it leaves ->qp with the old QP rather than NULL.  If pending RPC
tasks wake and exit, LOCAL_INV work requests will flush rather than
oops.

On initial connect, leaving ->qp == NULL is OK, since there are no
pending RPCs that might use ->qp. But be sure not to try to destroy
a NULL QP when rpcrdma_ep_connect() is retried.
